Health Requirements & Forms

All Kindergarten and Early Childhood students require a (Health Appraisal) physical examination by a private physician.  Also, required is an updated copy of immunizations for these students.

All new students and 1st, 3rd & 5th grade students require a physical exam before entrance into school in the fall. Parents should send this to the Health Office within thirty days of the start of the school year.

All students should submit an updated copy of immunizations if new immunizations have been given since last physical.  NYS now requires that Immunization Certificates must be signed by your Health Care provider.  An electronic signature is acceptable.

A dental certificate is requested for all new students and Kindergarten, 1st, 3rd & 5th grade.

All Early Childhood students need to provide a written statement signed by a health care provider that documents  Lead Screening at least 1 year of age but under 6 years of age. If a written statement of lead screening is not available from MD, documentation of the most recent LEAD LEVEL is acceptable.
